Plus: One-time enrollment fee of $50 applies (for monthly payment)

Terms & Limitations
This program is a discount plan, not a dental insurance plan. The annual premiums must be paid with either cash, check, or credit card. This plan is only valid at Smile on Westbury. It cannot be used at another dental office:
• In conjunction with another dental plan, dental insurance, or financing program such as CareCredit.
• For treatment which, in the sole opinion of our doctors, lies outside the realm of their capabilities.
• For referrals to a specialist
• For hospitalization or hospital charges of any kind
• For costs of dental care which are covered under automobile medical
• For services of injuries covered under workers’ compensation

Program Guidelines
•Patient’s portion of bill is due the day of service
• Cannot be used in conjunction with another dental plan.
• CareCredit may not be used to pay for the Club premium
• No refunds of premiums will be issued at any time if the participant decides not to utilize the Club plan.
